Consent & Waiver

In exchange for participation in the event with Little Moments Little Fam, I agree for myself and for the members of my family, to the following conditions:

I hereby certify that I am the adult parent, guardian, legal custodian, legal representative, physical custodian, or the person otherwise responsible for a minor child under the age of eighteen (18) years, and I consent to his/her participation in the activities offered by “Little Moments Little Fam”.

I understand that the risks associated with activities by Little Moments Little Fam could result in injury and/or death to my child. I hereby assume these risks and, knowing them, hereby give my child permission to participate. I understand that Little Moments Little Fam is not liable for any injuries or other occurrences due to indoor and outdoor activities or related risks, and/or the actions or omissions of Little Moments Little Fam staff, volunteers, employees, trustees, directors, officers, or any other entities being released.

I, as a parent or guardian, hereby voluntarily assume full responsibility for any loss or damage to property owned by me or my child, during participation in activities with Little Moments Little Fam.

I also pledge to absolve and protect Little Moments Little Fam from any claims, losses, liabilities, damages, or legal expenses, including court costs or attorney's fees, whether resulting from negligence on the part of Little Moments Little Fam or any other cause to the maximum extent permissible by law.

I further represent that the child(ren)/participant(s) are healthy and physically able to participate in any and all undertaken activities.

I understand that while participating in any Little Moments Little Fam activity, the SUPERVISION and CONDUCT of the child(ren)/participant(s) below is MY SOLE responsibility.

I agree that the child(ren)/participant(s) named below and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for play and participation in any play or event with Little Moments Little Fam

I agree to pay for all damages to the facilities caused by my or my family’s negligent, reckless, or willful actions. In addition, if I observe any hazard during our participation, I will bring it to the attention of the nearest employee or official immediately.

I recognize that there are certain inherent risks associated with the use of materials and accessories provided by Little Moments Little Fam and I assume full responsibility for personal injury to myself and my family members. I, for myself and the child(ren)/participant(s) named below, and our respective heirs, assigns, administrators, personal representatives, and next of kin, waive, release, and discharge Little Moments Little Fam and its property owner(s) along with their owners, agents, employees and affiliates, officers, and members, for injury, loss or damage arising out of or related to our participation in any and all of Little Moments Little Fam’s programs, activities, parties and/or the use of the facilities to the extent permitted by law. This waiver and release shall apply to all risks, known and unknown, even if resulting from negligent actions of other guests or employees of Little Moments Little Fam.

USE OF MATERIALS DISCLAIMER:

Although paints and other art supplies are claimed to be washable, we are not responsible for stained clothing and recommend that guests wear appropriate attire.
Little Moments Little Fam is not responsible for allergic reactions that may occur due to the use of materials during sensory classes, playgroups, or workshops.

PROPRIETARY MATERIALS:

I hereby agree not to disseminate, distribute, or use for any purpose other than intended, any materials received during any class or workshop conducted by Little Moments Little Fam.

USE OF PHOTOS AND VIDEO:

By participating in activities or events hosted by Little Moments Little Fam, I consent to Little Moments Little Fam's use of photographs and videos featuring my child, their caregiver, and any other attending children for promotional, marketing, and media purposes, should any such media be captured during the event.

I have read and understood the contents of this agreement, and I voluntarily sign it, intending to be legally bound by its terms. I acknowledge that this agreement constitutes a complete and unconditional release of all liability by Little Moments Little Fam to the greatest extent allowed by law.

This consent and waiver shall apply to all future events and activities with Little Moments Little Fam.
